Welcome to the vibrant community of St. Francis Xavier Church’s Youth Group! We have both Middle School and High School Youth Groups.  Our gatherings are a hub of fellowship, spiritual growth, and fun, designed specifically for our middle school and high school members.

To ensure a safe and organized environment for all, we require registration forms for each high school student wishing to participate. Here’s a simple guide to get started:

• Middle School Student? Middle School Students  Click here to register by filling all the Faith Formation Registration Forms at the bottom of the linked page.
• High School Student? This should be all public and Catholic School students in years one and two of Confirmation Preparation (usually 9th and 10th grades).  You first must fill out the five required forms listed below which includes: the Religious Education Registration Form, High School Youth Group Rider Form, Photo Release Form, Medical Authorization From, and Consent & Liability Form.

• High School Youth Group Rider Form 2026-2027 – To Go Along With Registration Form: This is your key to entering our welcoming Youth Group family.
• Photo and Video Image Release Authorization Form: Help us capture and share the joy of our gatherings while respecting everyone’s privacy and preferences.
• Youth Ministry Medical Authorization Form: Your health and safety are paramount to us during all Youth Group activities.
• Youth Ministry Consent & Liability Form: Ensuring a secure environment for all our youth, this form covers important consent and liability information.
